The record behind the answer
An Answer Is Not an Answer Without the Record
An inspector does not accept a procedure as an answer. They ask for the record that shows the procedure was followed, and for the data that shows the control worked. A script that names a document and stops there fails at the first follow-up question.
So we review the evidence, not only the programs:
Every answer in the script points to the specific record that supports it, by name and location, so the person answering can put a hand on it in the room.

The Engagement Where Everything Is Scored
Conformance is whether each document is right. Coherence is whether they are right together.

Conformance
Whether each assessed obligation is met.

Coherence
Whether the programs hold together where they touch. The questions cut across programs, and an answer that satisfies one program while contradicting another is the answer that draws the follow-up.

Effectiveness
Whether the program is working, scored from the operating evidence itself. Where a record is the only evidence available, the score says so.
Incidents happen at the seams, and so do findings. Because a Readiness Assessment goes into the execution evidence behind each answer, Effectiveness can be scored here, from the evidence behind each answer.

Each stands on its own. A Readiness Assessment sets its own applicability determination and scores all three components as of its own date.
