Week ending October 18, 2026
Each week we flag the developments that actually touch the requirements pipeline operators and their contractors manage, across federal rules, state programs, and industry standards. No commentary and no spin: just the item, the requirement it affects, and a direct link so you can read it and decide for yourself.
Federal Rules & Federal Register
We found no PHMSA Federal Register document that changed a pipeline safety requirement in this window. No PHMSA document of any kind was published between October 12 and October 18, 2026, and none has been published since the September 28 hazardous materials final rule carried two issues ago. The most recent PHMSA pipeline safety document remains the September 22, 2026 notice of the October 21 Liquid Pipeline Advisory Committee meeting. (Federal Register PHMSA agency page)
Three PHMSA notices are scheduled for publication Monday, October 19, 2026, the day after this window closes: Notice of Applications for New Special Permits, Notice of Applications for Modification to Special Permits, and Notice of Actions on Special Permits. All three sit in the hazardous materials special permit program under 49 CFR Part 107 rather than pipeline safety, and they will be carried in next week’s issue if anything in them reaches a pipeline requirement. (Federal Register, new special permits; modifications; actions)
The October 21 Liquid Pipeline Advisory Committee meeting is unchanged and registration for it closed inside this window, on October 17, 2026. The meeting remains October 21, 2026, 9:00 a.m. to 3:00 p.m., virtual, with post-meeting written comments due November 20, 2026. The agenda remains three published hazardous liquid NPRMs, each on its own docket: Breakout Tank Inspection (Docket PHMSA-2025-1271), Remote Monitoring of Hazardous Liquid Pipeline Rectifiers (Docket PHMSA-2026-1550), and Hazardous Liquid Valve Maintenance Schedule (Docket PHMSA-2026-1554). (Federal Register; 91 FR 60190; Docket PHMSA-2026-2773) Relates to: 49 CFR 195.3, 195.205(b) and 195.432(b); 49 CFR 195.573(b)(2) and 195.573(c); 49 CFR 195.420(b); and the advisory committee procedure at 49 CFR 190.9.
No extension, reopening, or decision has been published on the Gexcon U.S., Inc. petition docket since comments on EFFECTS Version 12.2.0 as an alternative model for the LNG flammable vapor-gas dispersion exclusion zone closed October 9, 2026. (Federal Register; 91 FR 57446; Docket PHMSA-2024-0164) Relates to: 49 CFR 193.2059 and the petition procedure at 49 CFR 190.9.
PHMSA Guidance & Interpretations
No new PHMSA advisory bulletin, letter of interpretation, or guidance item was posted in this window. Two entries dated inside the window were added to PHMSA’s interpretations register, references 26-0094 (Pacific Gas & Electric Company, October 13, 2026) and 26-0103 (Thompson Gas, October 14, 2026), along with 25-0128 (Fike Corporation, October 13, 2026); all three sit in the hazardous materials letters of interpretation register rather than pipeline safety, the register index does not publish the regulation sections each one reaches, and each entry’s own page carries an earlier effective date than the index date shown here, so treat the October dates as posting dates rather than issue dates. PHMSA’s guidance portal still shows its most recent item as July 13, 2026 (“Guidance for Enhancing the Effectiveness of Distribution Integrity Management Programs”), unchanged for fourteen weeks. PHMSA’s newsroom posted nothing inside the window; its most recent item remains the September 30 grant announcement. (PHMSA Guidance; PHMSA Letters of Interpretation; PHMSA press releases)
One pipeline interpretation remains pending on PHMSA’s pending-interpretations page, PI-26-0006, requested by Martin Operating Partnership LP on the definitions of service line, distribution line, and transmission line. It was posted September 9, 2026 and its comment period closed October 9, 2026. (PHMSA Pending Pipeline Interpretations) Relates to: the definitions at 49 CFR 192.3.
State Programs
No new state pipeline-safety rule change that alters a tracked requirement was identified in this window. State registers were not comprehensively polled this run; treat this section as not confirmed.
Enforcement & NTSB
No new PHMSA enforcement action or NTSB pipeline item dated in this window was identified. The week’s substantive enforcement item is a corrective action order issued three weeks earlier that reached the trade and local press inside this window.
PHMSA has ordered Energy Transfer, LP to inventory and remediate steel encirclement repair sleeves across its hazardous liquid pipelines, following a September 18, 2026 crude oil leak in Sterling Heights, Michigan. (PHMSA Corrective Action Order, CPF 4-2026-025-CAO)
At a glance
- Applies to: HL. The order’s own scope reaches the Marysville to Toledo Crude Pipeline and, for the sleeve provisions, “Affected Pipelines,” defined as hazardous liquid pipelines owned or operated by Energy Transfer and its subsidiaries and related entities.
- What it is: A corrective action order issued without prior notice or opportunity for hearing. The order describes a leak discovered September 18, 2026 in Sterling Heights, Macomb County, Michigan, confirmed the following day as a failed Type B repair sleeve on the 16-inch, 1967-vintage, low-frequency ERW seam Marysville to Toledo Crude Pipeline, roughly 130 miles from Marysville, Michigan to Toledo, Ohio. It directs that the isolated segment not operate until the Director authorizes restart, imposes a 20 percent pressure reduction on the affected segment, and requires a written restart plan, mechanical and metallurgical testing, a root cause failure analysis, a system-wide inventory of steel encirclement repair sleeves, and a sleeve remediation plan. Local reporting adds that Energy Transfer has requested a hearing and that no date had been set; the order itself does not record a hearing date.
- Key date(s): Leak discovered September 18, 2026; sleeve failure confirmed September 19, 2026; order issued September 21, 2026. Metallurgical testing due 45 days after receipt; root cause failure analysis due 90 days after receipt; first quarterly report due November 1, 2026.
- Relates to: the corrective action order authority at 49 CFR 190.233 and the hearing procedures at 49 CFR 190.211; the integrity management provisions at 49 CFR 195.452, including the seam-assessment provision at 195.452(c)(1)(i); the high consequence area definitions at 49 CFR 195.450; and the records provisions at 49 CFR 195.404(c)(1) and 195.214(b).
PHMSA’s published enforcement extract did not refresh this week and holds the stamp it took on last week. Its source stamp still reads “PHMSA Work Management System as of 10/02/2026,” now in its second week. The dataset is refreshed monthly. PHMSA’s latest-actions page shows no enforcement item dated inside the window. (PHMSA Enforcement Data; PHMSA Latest Actions) Relates to: the enforcement procedures at 49 CFR Part 190 and the reporting requirements at 49 CFR Part 191.
A trade compilation published October 12, 2026 reports that PHMSA issued four corrective action orders, two notices of amendment, one warning letter, and five notices of probable violation in September 2026, with $1,953,719 in total proposed civil penalties; the largest single proposed penalty listed, $818,778, is tied to 49 CFR 195.402(a), procedure manuals. Reported by RCP; the compilation names no operators or CPF numbers, and the individual case records were not matched to it this week. (RCP) Relates to: 49 CFR 192.605(a) and 195.402(a) (procedure manuals), 49 CFR 192.617 (incident investigation), and 49 CFR 195.444 and 195.446 (leak detection and control room management).
NTSB posted no pipeline release inside the window, and no release of any kind; its most recent item remains the September 21, 2026 release on rail worker protection during shoving movements. The preliminary report on the July 28, 2026 Union, Michigan gas distribution explosion, carried in prior issues, remains reported by local and trade press dated around August 19, 2026; a primary NTSB document for that event has still not been located. (NTSB News)
Standards & Editions
No new edition, addendum, errata, or public-review item dated in this window was identified. API’s standards announcements page still shows the second edition of API RP 1176, Assessment and Management of Cracking in Pipelines, announced August 18, 2026, as its most recent pipeline item, unchanged for a ninth week. The page’s most recent entry overall is the third edition of API Specification Q2, announced September 10, 2026, which covers well-site service quality management rather than pipelines. (API Standards Announcements)
Calendar reminder, not new this week: the consensus-standard edition updates PHMSA issued July 31, 2026 take effect January 1, 2027, when the updated NACE, NFPA, ASTM, and MSS editions incorporated by reference into 49 CFR Parts 192 and 195 become the versions operators must work to. That is now ten weeks out. Those updates were issued one standard per notice; linked here is the NACE SP0502 notice from that batch. (PHMSA standards update, NACE SP0502, July 31, 2026)

Industry News
Only news items that map to a requirement we track; same rules (headline + source + link + tag, no commentary).
The September 21 corrective action order to Energy Transfer, LP, covered under Enforcement above, reached wider reporting on October 15, 2026. Local coverage ties it to a second sleeve failure, an estimated 6,500 gallon jet fuel release discovered in January 2025 on the Twin Oaks pipeline that contaminated groundwater under the Mount Eyre neighborhood in Upper Makefield Township, Pennsylvania, and reports that 45 Type A sleeves were installed on that line, that repair records for the failed sleeve and four others could not be located, and that PHMSA has requested sleeve information for pipelines in ten states. Those three figures are from press reporting and were not matched line by line to the order text. (Bucks County Herald; Hoodline) Relates to: 49 CFR 195.404(c)(1) and 195.214(b) (repair records and welding procedure retention) and 49 CFR 195.401(b)(1) (correction of unsafe conditions).
On the September 18, 2026 rupture and fire on a 16-inch Texas Gas Transmission line near Jennings, Louisiana, carried in prior issues, no PHMSA enforcement record, corrective action order, or incident record has appeared. The item still rests on a single trade-press source. (Underground Infrastructure)
On the two October 6, 2026 New Mexico pipeline fires carried in last week’s issue, the fatal oilfield pipeline fire in Lea County and the Harvest Midstream rupture near La Boca in San Juan County, no PHMSA, New Mexico Oil Conservation Division, or other regulator record has been located for either event, and the operator and commodity on the Lea County line remain unreported. (KCBD; KOB 4)
On the Sable Offshore Corp. special permit matter carried in prior issues, no new filing or order in the Ninth Circuit proceedings was identified inside this window, and nothing further has been added to the PHMSA docket. (PHMSA special permit request notice, Docket PHMSA-2026-0464)
On the federal pipeline safety reauthorization, the House bill H.R. 9338, the Pipeline Safety Authorization Act of 2026, records no action since July 21, 2026, when the Energy and Commerce Committee ordered it reported in the nature of a substitute by a vote of 41 to 8; no floor action occurred inside this window. (H.R. 9338 bill status)
